Office Appointment Details
New Clients
For new clients only, if you have not already completed the New Client Form , please do so before your appointment.
The New Client Form is only required for your first appointment with Paragon PsychConnect.
If you are unable to complete the New Client Form , please arrive 10 minutes earlier than your appointment time to fill in the paper form.
Appointment Conditions
Your initial appointment will be 60 minutes (you may also be asked to complete some questionnaires). All following appointments will be 50 minutes.
Please remember to arrive 10 minutes prior to your initial appointment.

Initial appointments cancelled without due notice (at least 24 hours) or when you receive the SMS appointment reminder will not be re-booked as our experience has shown us that if people can’t make their first appointment a priority, they will be poor attenders.
Full fees will be charged if we do not get at least 24 hours’ notice of cancellation for all appointments.
NOTE: SMS and email reminders are a courtesy only – please remember your appointments with us without the need of a formal reminder.

COVID-19
We take your health seriously and will be taking all precautions in our Practice.
Appointments are face to face unless a Telehealth consult has been arranged.
If you are unwell, or have any COVID-19 symptoms, or you prefer to attend the appointment remotely, please contact us on (08) 7095 3484 and we can arrange a phone or ZOOM consult.

Car Parking
Our car park is directly behind our building and has Paragon PsychConnect parking signs.
If there are no remaining car parks in our car park, please drive out the rear car park exit, turn right, and park in the Minicozzi Shopping centre rear car park.
Disabled car parking is available at the front of our building.
Please do not park in the adjoining business car park at 301 Payneham Road.

What to Bring
Please bring with you any items as listed:
- Your doctor's referral
- Claim number if your condition is compensable or
- Aged Pension / Unemployment / Disability card if it is a Private Concession
- Any current medical reports that you have in relation to your condition
- Reading glasses if you need them

Private Client (Medicare and Private) Fees

* If you are claiming the concession rate, you need to be receiving the Aged Pension, Job Seeker allowance or Disability payments. Please bring your Centrelink concession card.
** Please note that Medicare Rebate will depend on whether you have reached your Medicare Safety Net Threshold.
Report and Letter Fees
Reports and letters requested by clients (e.g. Centrelink, NDIS application, Legal, other) need to be negotiated and will be charged per hour.
