Support Coordination Appointment Details

You have been directed to this page because an appointment was made to see a Paragon PsychConnect therapist as arranged with your therapist.

New Clients

For new clients only, if you have not already completed the New Client Form , please do so before your appointment.

The New Client Form is only required for your first appointment with Paragon PsychConnect.

If you are unable to complete the New Client Form , please arrive 10 minutes earlier than your appointment time to fill in the paper form.

Appointment Conditions

We will come to your home or to another site if we have negotiated this at the time of booking the appointment.

If you are unable to keep this appointment, please contact us as soon as possible by phone:  (08) 7095 3484 or email.

Initial appointments cancelled without due notice (at least 24 hours) or when you receive the SMS appointment reminder will not be re-booked as our experience has shown us that if people can’t make their first appointment a priority, they will be poor attenders.

Full fees will be charged if we do not get at least 24 hours’ notice of cancellation for all appointments.

NOTE: SMS and email reminders are a courtesy only – please remember your appointments with us without the need of a formal reminder.
