This page lists the Paragon PsychConnect forms which are used for various purposes.

You can choose to fill in the Online Form (web page) or the PDF Form version of the forms.

The PDF Form will open in a new web browser tab. You can then either fill in the form in the web browser and Save it, or you can Save it and fill in the saved form later.

If you are filling in the PDF Form version of the form, please use this Email link to open a new email to us. This will allow you to attach the saved completed PDF Form and send the email to us.

Note that the Email link will only work if you have email installed on your PC, or laptop, or mobile device.

New Client Forms

For new Paragon PsychConnect clients only.

The following New Client Forms are used for new Paragon PsychConnect clients before their first appointment.

Please do not use these forms until after you have made your first appointment with the Paragon PsychConnect team.

You can choose to fill in the Online (website), or PDF, versions of the New Client Form.

If you are filling in the PDF Form, please send the form to us by Email, or print the form and bring it with you to your initial appointment.

If you are unable to complete the Online, or PDF, version of the New Client Form, please arrive 10 minutes earlier than your first appointment time to fill in the paper form.

Referral Forms

The following Referral Forms are used to refer Compensable, NDIS, Medicare, and Private Health clients to Paragon PsychConnect.

You can choose to fill in the Online, or PDF, versions of the Referral Forms.

If you are filling in the PDF Form, please send the form to us by Email, fax, or mail.

If you are unable to complete the Online, or PDF, version of the Referral Form, you can Email, fax or ring us to make an appointment with one of our Team Members.
